Launch Consent Agreement
The agreement you accept when asking xthrive to contact you about the Thrive launch and enrolment.
What I am agreeing to+
By selecting the consent checkbox and submitting the launch form, I voluntarily ask xthrive to store my name, email address and optional mobile number and to contact me individually about the launch of Thrive and possible enrolment.
Purpose and limits+
My details may be used for launch announcements, availability updates, enrolment information and direct follow-up connected to Thrive. They may not be sold or used for unrelated advertising.
No clinical relationship yet+
Joining the launch list does not enrol me as a patient, establish a clinician–patient relationship, provide medical advice or guarantee acceptance into the Thrive service for diabetes, hypertension or obesity. The platforms are not yet available for clinical use.
No health information in this form+
I understand that I should not submit diagnoses, symptoms, readings or other medical information through the launch form or by replying to a launch notification unless xthrive later provides an approved secure clinical channel.
Withdrawing consent+
I may withdraw this consent at any time by emailing dunisani@fastcareonline.co.za. xthrive will stop future launch communications and remove information that is no longer lawfully required.
Record of consent+
xthrive records the date, purpose and version of this agreement accepted with my submission so that my consent can be demonstrated and managed.
